IN GBUZ «clinical hospital № 5 of the city of Vladimir» organizes and conducts a medical examination of foreign citizens to obtain a medical report on the absence or presence of diseases that pose a danger to others and is grounds for denial or revocation of a temporary residence permit or a residence permit or work permit in the Russian Federation, or patent.
To pass a medical examination, a foreign citizen or a stateless person submits the following documents to the institution:
1) the identity documents:
a) the documents certifying the identity of a foreign citizen in the Russian Federation are the passport of a foreign citizen or other document established by Federal law or recognized in accordance with an international treaty of the Russian Federation as a document certifying the identity of a foreign citizen;
b) the documents certifying the identity of a stateless person in the Russian Federation are:
— a document issued by a foreign state and recognized in accordance with an international treaty of the Russian Federation as a document certifying the identity of a stateless person;
— temporary residence permit;
— residence permit;
— other documents provided for by federal law or recognized in accordance with an international treaty of the Russian Federation as documents certifying the identity of a stateless person;
2) a migration card and its copy (if available) for foreign citizens and stateless persons who arrived in the Russian Federation in a manner that does not require a visa;
3) the visa and its copy (for foreign citizens and stateless persons who arrived in the Russian Federation in the order requiring a visa).
According to the results of the medical examination, a medical report is issued on the presence (absence) of drug addiction, infectious diseases that pose a danger to others and are the basis for refusing or revoking a temporary residence permit for foreign citizens and stateless persons, or a residence permit, or a patent, or a work permit in the Russian Federation.
* Medical examination is carried out within a period not exceeding 5 working days from the date of application of a foreign citizen or a stateless person.
* The medical report is valid for three months from the date of issue.
